This programme allows you to plan and record your activities and appointment dates and allows you and your doctor to understand how you are benefiting.

You don’t need to register to participate in the programme, but if you do, it enables you to record your data securely online and view your progress over time. Alternatively, if you prefer not to register, you can still complete the tracker documents online and save them to your computer, or simply download and print the documents to complete later.

The tracker comprises of four parts:

Use the tracker to log your activity and treatment. It will help you see how they are benefiting you, so that you stay motivated and active.

STEP 1: SELF-EVALUATIONS

Keep track of your health and symptoms periodically, so that you can notice improvements and provide key information
to your doctor.

STEP 2: APPOINTMENT LOG

Record the details and test results from your doctor’s appointments to track how testosterone therapy is benefiting you.

STEP 3: ACTIVITY PREFERENCES

The purpose of this step is to help you choose the activities that you would consider adding to your weekly activity planner when you feel ready to increase your level of physical activity.

STEP 4: ACTIVITY PLANNER

Plan your physical activity and record your progress for up to six weeks, until activity is established as part of your routine, to maximise the benefits of treatment.
